Ages and Stages Developmental Questionnaire:  If your child is coming in for 9 months, 1 yr, 2 yr, or 3 yr  well-child check up, please print and fill out appropriate questionnaire prior to coming in.  (If your child is younger than 10 months, fill out the 8 months questionnaire).

 For all new patients, please bring us immunization record if you have it, and any significant past medical records.  For newborns, be sure to bring us the nursery discharge paper and PKU lab slip with your first newborn visit to our office

Before your initial visit, please download the Registration Form below, complete it, and bring it with you.  This will help us to be able to see your child more timely. Please arrive 10 minutes prior to scheduled appointment for first time visits.
