Newborns: You need to call the insurance carrier/Human Resources at your work and add your son/daughter to your plan.  Be sure to start the coverage from the child’s date of birth.  They will need to know that Dr. Janet Jung will be his/her pediatrician .  This should be done as soon as possible; otherwise you may be billed for your initial visit with us.

Transferring Patients: Contact your insurance carrier and change your child’s pediatrician to Sweetwater Pediatrics, Dr. Janet Jung.

Sweetwater pediatrics offers comparable pricing for your child’s care.  We accept cash, Master Card, VISA, Discover, American Express or personal check for your out-of-pocket fees.  If you are self-pay patients, we have self-pay fee schedule.  Please call our office to ask about it.

Be sure to check with your insurance company regarding your responsibility for co-pays, deductibles, and co-insurances.  We will be glad to check for you if you have problems obtaining the information.  Please let us know at the time of the visit if there are any changes with these amounts or if your insurance changes.   Financial Policies
